Oxytetracyclines (engemycin) can be used in guinea pigs but they are definitely considered a 'use with caution' drug (according to the books by Richardson and Quesenberry referenced by GL which I happen to have here) - I certainly advise her to be on probiotics at the same time while she's on it. However, ultimately the risk of GIT stasis needs to be weighed up against the potential benefit in terms of fighting the infection. You're best to speak to the vet if you are concerned about this.
Considering the infection has not responded to baytril and surgery is not considered an option (was his opinion about her not making it thru the surgery based on her stability/ill health?), and the moderate risks involved in the use of oxytet, bacterial culture & sensitivity testing was another option that could have been looked at (however this usually takes 3-5 days...).
At this stage I wouldn't necessarily advise you to discontinue the antibiotics. It sounds like she's starting to really go downhill and the risk is probably worth it.

Fleece would be fine imo, you could even just layer it over the sawdust if you like. That should be fine for keeping the wound clean. Just make sure to watch for any discharge, excessive swelling or irritation around the incision site and call the vet if you are at all concerned. Most vet clinics do offer free post operative checks to make sure it's all healing as planned. I certainly don't mind seeing patients come in for rechecks at all!
I'm glad to hear she's having the op! Hopefully it goes well and she makes a full recovery.
